- Log in to Blue Connect
- Go to Manage Plan
- Scroll to Link a Plan
- Enter the requested information
- Click Submit

Get personalized care
It’s important to have an in-network doctor you see regularly. Your primary care provider (PCP) handles most of your medical needs, including routine tests and preventive care, which are 100% covered.
To help you achieve the best health possible, your PCP will work together with any other doctors or specialists you see. That’s why the first thing you should do on Blue Connect is select an in-network PCP. Don’t wait until you’re sick. Choose your doctor now, so you’re all set when the need arises.
Here's how it works:2
- Make an appointment with your in-network PCP for a wellness visit.
- Have your annual wellness visit.
- Ask for preventive care screenings and tests that are 100% covered by your plan.
Once your claim is processed, you can go online to get your $25 gift card in the Blue Connect Activity Center. Please allow 4–6 weeks for this wellness visit reward to process.
- Log in to Blue Connect
- Scroll to the Primary Care Provider section
- Click on Choose Primary Care Provider
- Search in-network doctors to find your current PCP or to select a new one
- Once located, click Select as Your PCP
- Click OK to submit your selection

Your doctor can provide a lot of helpful information, but sometimes it can be hard to remember what to do once you leave the doctor’s office or between visits. This is where the Nurse Support Program can help.
