Healthy Blue + Medicare℠ (HMO-POS D-SNP) makes it easier for you to get the care you need, when you need it, at a low cost. You get full Medicare and Medicaid benefits, plus extra services like care coordination and food support.1
With your Healthy Blue + Medicare plan you have access to:
Managing your health care alone can be hard, especially if you have multiple health problems.
You'll have a case manager who will work with you and your providers to make sure you get the right care. Our case managers are nurses, social workers, and mental health clinicians.
You get access to extra benefits that can help you meet your health goals. Benefits include:
SafeRide℠ Health makes it easy to get to and from doctor appointments, your pharmacy, and the grocery store.1 SafeRide matches you with the transportation type that meets your needs, including wheelchair accessible vans and non-emergency ambulances. You can book in advance or up to two hours before your appointment. This benefit includes 48 non-emergency, one-way rides per year.5

If you have severe chronic conditions or are at risk for hospitalization, you may qualify to receive extra, non-medical benefits called Special Supplemental Benefits for the Chronically Ill (SSBCI).1
These benefits are available for members who qualify, and can include extras like groceries, transportation, and more.
To learn if you are eligible for SSBCI, call us at 833-713-1078 (TTY 711), 8 AM to 8 PM ET, seven days a week.
