Improve your quality of life

The Blue Cross and Blue Shield of North Carolina (Blue Cross NC) Nurse Support Program gives you one-on-one support from a team that may include nurses, social workers, dietitians, and pharmacists. Together, they work with you to help you better manage your care and feel more confident in your health decisions.

If you're dealing with complex conditions, multiple health issues, accident‑related injuries, disease‑specific illnesses, or short‑term disabilities, a care manager nurse can help you:

  • Understand your condition and treatment options
  • Make informed decisions about your health care
  • Learn methods to improve how you manage your condition day to day
  • Understand your benefits and access programs available from your plan
  • Connect with community resources that offer additional support

This program is available at no cost for members with family or individual plans.

How the Nurse Support Program works

You’ll be connected with a registered nurse who will guide and support you throughout your care. Your nurse acts as your advocate, helping you understand your options and making sure your needs are addressed.

The Nurse Support Program doesn't replace your current care. Instead, your nurse will partner with you and your doctors to create a personalized care plan designed around your specific needs. They can help you understand your condition, manage your treatment, and connect you to helpful resources.

Your nurse will continue to work with you until your condition is improved or you’ve reached a point where you can more easily manage your own care.
