Get one $50 reward for completing your preventive care visit. You can earn your reward for completing one of these three types of preventive care visits:
- Annual Wellness Visit
- Welcome to Medicare Visit
- Annual Routine Physical
Additional rewards for members with diabetes
If you are managing diabetes, routine tests and exams are crucial to maintaining your health. Get an additional $100 in rewards for keeping up with diabetes care.1 2
- Retinal eye exam ($50 reward) – a full eye exam with dilated pupils (you can find a vision provider via the Blue Cross NC member portal)
- A1C ($25 reward) – a blood test to measure blood glucose (sugar)
- Kidney health evaluation ($25 reward) – blood/urine tests to measure function
This $100 in rewards can be combined with your Annual Preventive Visit for a total of $150. During your Annual Preventive Visit, talk with your doctor about these and other screenings you may need.
Why preventive care is so important to your health
Your primary care provider (PCP) knows your health care needs best. That’s why scheduling your Annual Wellness Visit,3 Welcome to Medicare Visit,4 or Annual Routine Physical with your PCP is so important. Whether your PCP is a doctor, nurse practitioner, or physician assistant, they work with you to:
- Assess and monitor your health
- Conduct screenings and administer essential vaccines
- Discuss your health risk factors and strategies to best manage them
- Schedule this visit with your PCP.
- Specifically mention that you are scheduling an Annual Wellness Visit, Welcome to Medicare Visit, or Annual Routine Physical in order to qualify for your $50 preventive care reward.
- To ensure you and your provider can focus on preventive care, make sure you aren't sick or in pain on the day of your preventive visit. If you are, communicate this with your provider so they can treat what ails you. Be sure to reschedule your annual preventive visit for another day.
- Talk with your PCP about any challenges you’re having with mobility, balance, or risk of falling, and ask about ways to stay safe and active.
- Mention if you’ve noticed occasional leaks or changes in bladder habits even if they seem minor.
- Share how your health impacts your ability to complete everyday activities — such as bathing, dressing, preparing meals, or managing medications — so you can get the support you need.
- Ask about ways to maintain your physical and emotional well‑being, including managing pain, staying active, and reducing stress.
- Review any concerns about your cognition or memory, and ask whether a screening may be helpful.
How to claim up to $150 in rewards
- Attend your preventive care visit and diabetes care screenings.
- Log in to the Blue Cross NC member portal.6 Scroll down until you see the orange Blue FlexCard section, then select View balances.
- On the Blue FlexCard site, select See more under 2026 Rewards & Incentives. Look for an activity form in your Quick Links.
- Complete the form, if it’s there. (If it’s not, you've already completed all eligible rewards. See the rewards you’ve earned below.) Soon after submitting the form, up to $150 will be added to your Blue FlexCard.7
