Medicare doesn’t have to be overwhelming, even when you’re figuring it out for someone else. If you're helping a loved one choose a plan, this guide walks you through the Medicare basics and how to approach the conversation, plus questions to ask and local resources you can lean on.

Medicare basics every caregiver should know

Let’s start with the basics: what is Medicare? Medicare is a federal health insurance program for people 65 and older, and for some younger people with certain health conditions. It’s organized into parts, and each one covers a different kind of care.

  • Part A helps cover hospital stays, skilled nursing care and hospice.
  • Part B helps cover doctor visits and outpatient care - care you get without being admitted to a hospital.
  • Part C, also called Medicare Advantage, is an all-in-one plan option offered by private insurers.
  • Part D helps cover prescription drugs.

There's a few ways to get Medicare coverage. With Original Medicare, your loved one gets Part A and Part B through the federal government. From there, they can add a separate Part D plan for prescriptions, and many people also add a Medicare Supplement plan, sometimes called Medigap, which helps cover some of the out-of-pocket costs Original Medicare doesn't. Another option is Medicare Advantage, also called Part C, where a private plan combines coverage into one package, often with prescription drug coverage, dental, vision, and fitness.

Questions to ask before comparing Medicare plans

Once your loved one is ready to look at their options, a little prep goes a long way and learning a few details first makes it much easier to compare plans side by side. Before comparing anything, try and learn:

  • Which doctors do they want to keep? List their current doctors and specialists. Medicare Advantage plans use a network of doctors and hospitals, though how strict that network is varies by plan. HMO plans generally require staying in network for care, while PPO plans offer more flexibility to see providers outside of it. So, check that their doctors are covered for any plans they’re considering. Original Medicare and Medigap don’t use networks, so they can see any provider that accepts Medicare.
  • What prescriptions do they take? Note the names and doses. The price of a medication can vary by plan, so it's worth double checking to make sure your list is right.
  • What’s their budget? The monthly premium matters, but it’s not the full picture. Look at the deductible, copays, and coinsurance, too, so you can get a clearer sense of the full cost before deciding.
  • How do they like to receive care? Medicare Advantage plans typically cover care within a network, while Original Medicare and Medigap work with any provider that accepts Medicare, nationwide. If they travel often or split time between states, that’s worth factoring in.

Pulling all the details together is the biggest step. Once you have them, comparing plans gets a lot easier. And the real question is simple: which plan fits the life they actually live? There usually isn't one "best" plan, only the one that fits them best.

How to talk to your loved one about Medicare

For many families, the hardest part isn’t the paperwork. It’s starting the conversation. Talking about health and money can feel personal, and outside of many people’s comfort zones. A few things can help make this conversation easier:

  • Pick a calm moment. A quiet afternoon tends to work better than in the middle of a stressful day or a big family gathering.
  • Lead with them, not the task. Try “I want to make sure you have everything you need” instead of “We have to figure out your insurance.”
  • Listen first. Ask what matters most to them before offering solutions.
  • Go at their pace. You don’t have to decide everything in one sitting. Sometimes the first conversation is just about opening the door.

You and your loved one are working toward the same goal: finding coverage that supports their health, their needs and the life they want to live. Keeping that shared purpose in mind can make the conversation feel less stressful and more supportive.

Medicare help for caregivers in North Carolina

  • Beyond Blue Neighborhood Centers: Sometimes it’s easier to talk about things face to face.   • Care Managers: If your loved one is a Blue Cross and Blue Shield of North Carolina (Blue Cross NC) member, they may also have access to a care manager. These are trained professionals who help you and your loved one think through questions and point you toward the right resources. They’re a knowledgeable, local starting point when you’re not sure what the first step is or what questions to ask.
  • Extra Caregiver Support: Depending on plan and eligibility, some Blue Cross NC members can access extra caregiver support through partner organizations that provide guidance and resources. If your loved one is already a member, it’s worth asking what kind of support might be available.

Caregiving asks a lot of you, and learning the ropes of Medicare might be part of it.
