Important credentialing update: Statement of Supervision and Locum Tenens forms no longer accepted
Effective immediately, Blue Cross and Blue Shield of North Carolina (Blue Cross NC) no longer accepts or requires Statement of Supervision (SOS) and Locum Tenens forms for credentialing- or enrollment-related processes. These forms should no longer be submitted.
Practitioners seeking credentialing with Blue Cross NC should instead complete the following steps to initiate the credentialing process:

Ensure your CAQH application is complete and in a “Completed” status.

Authorize Blue Cross NC to access your CAQH information.

Email the Blue Cross NC Credentialing team with your:
- Practitioner name
- NPI
- CAQH ID
Once this information is received, the Credentialing team will retrieve your CAQH information and begin the credentialing process. Credentialing applications that are complete and accurate are typically processed within 20 days or less. Practitioners who submit an SOS form will be directed to follow the CAQH-based credentialing process instead.
References to the SOS and Locum Tenens forms will be removed from the Provider Blue Book during its next scheduled update.
