Update: Reminder Annual Provider Training and Attestation Requirements (Healthy Blue + Medicare (HMO D-SNP))

Update March 10, 2025: We are pleased to announce that Model of Care (MOC) training for Blue Cross and Blue Shield of North Carolina (Blue Cross NC) Medicare Advantage providers is now available on Blue e Related Links. The new availability of the MOC training on Blue e is in addition to the continued training availability on the Healthy Blue + Medicare page of the Blue Cross NC website.  As a reminder, all participating providers in our Blue Cross NC Medicare Advantage network are required to complete the Healthy Blue + Medicare (DSNP) Model of Care (MOC) training annually.

Please note: This communication applies to Healthy Blue + Medicare^℠ (HMO D-SNP) offered by Blue Cross and Blue Shield of North Carolina (Blue Cross NC).

This is a reminder that all participating providers in our Blue Cross and Blue Shield of North Carolina Medicare Advantage network are required to complete the Healthy Blue + Medicare (DSNP) Model of Care (MOC) training. Centers for Medicare and Medicaid (CMS) require all payers that offer a SNP product to provide Model of Care training for their network providers. This training is required annually. In our November 5, 2020 communication, we stated that our current Medicare Advantage provider network would be included in the DSNP product and that no additional contracts were required to be signed for participation.

Failure to complete annual MOC training can result in corrective action, which may include suspension or termination from the network.
