Annual provider training and attestation requirements for 2026 (Healthy Blue + Medicare (HMO D-SNP))

Please note: This communication applies to Healthy Blue + Medicare^℠ (HMO D-SNP) offered by Blue Cross and Blue Shield of North Carolina (Blue Cross NC).

This is a reminder that all participating providers in our Blue Cross and Blue Shield of North Carolina Medicare Advantage network and non-participating providers that routinely see D-SNP members, are required to complete the Healthy Blue + Medicare (D-SNP) Model of Care (MOC) training. The Centers for Medicare and Medicaid (CMS) require all payers that offer a D-SNP product to provide MOC training for their network providers. This training is required annually.

In our November 5, 2020, communication, we stated that our current Medicare Advantage provider network would be included in the D-SNP product and that no additional contracts were required to be signed for participation.

The MOC training is available on the Healthy Blue + Medicare page of the Blue Cross NC website as well as on Blue e under Related Links.

Failure to complete annual MOC training can result in corrective action, which may include suspension or termination from the network.

Please note that MOC training must be completed for providers to participate in their members’ Individualized Care Plan (ICP) and Interdisciplinary Care Team (ICT) meetings. Provider support is critical to ensure effective care coordination, improved quality, and positive health outcomes.
