Enhanced Plan
No deductible
$10 and $30 copayments for most drugs
No "coverage gap" for generic drugs
Only 5% coinsurance after $4,050 in total out-of-pocket costs
A broad range of covered drugs
A large and accessible pharmacy network

Standard Plan
$275 deductible
25% coinsurance until "coverage gap" begins
No coverage during coverage gap
Only 5% coinsurance after coverage gap ends at $4,050 in total out-of-pocket costs

To enroll, complete the following steps:

Download and print the enrollment form by clicking this link:  Blue Medicare Rx Plan enrollment form  Complete all sections of the form following the instructions outlined on the form. Mail the completed form to the following address: BCBSNC Enrollment P.O. Box 17168 Winston-Salem, NC 27116 You should receive your membership information, including your BCBSNC ID card, 10 to 14 days after your application has been received. If you need assistance filling out the application, please call a Direct Sales authorized agent toll-free at 1-800-478-0583, 7 days a week, from 8:00 a.m. to 8:00 p.m.
