Take charge of your health and save money with Blue Extras, BCBSNC’s group of value-added programs, including discounts on a variety of health-related services. In addition, you have access to registered nurses 24-hours a day through Health Line Blue. Audio Blue, Optic Blue and Cosmetic Surgery Blue are only available in North Carolina at this time. |

| Prior Plan Approval |

Certain medical services may require prior approval before the services are utilized. All services for mental health and substance abuse need to be approved through CBHA at 800-475-7900. Some prescription drugs may have specific approval guidelines. Contact the plans' pharmacy administrator, Catalyst Rx, at 888-869-4600 or visit their website at www.catalystrx.com for information specific to prescription drugs. |

| Explanation of Benefits (EOB) |
|
View Blue Cross Blue Shield of North Carolina's policy on when Explanation of Benefits (EOBs) are mailed. EOBs that are not normally mailed are available for viewing and printing on the Member Services link or can be requested by calling Customer Service. |

You can complete a variety of self-service tasks online, 24 hours a day, including checking the status of claims and requesting a new ID card. It is recommended that you provide changes in contact information directly to the Wake Forest University Human Resources Office. |
