Certain medical services may require prior approval before the services are utilized. All services for mental health and substance abuse need to be approved through CBHA at 800-475-7900. Some prescription drugs may have specific approval guidelines. Contact the plans' pharmacy administrator, Catalyst Rx, at 888-869-4600 or visit their website at www.catalystrx.com for information specific to prescription drugs.
