Medical Guidelines	Reason for Update
Left Atrial Appendage Closure Device for Stroke Prevention	New policy implemented. Left atrial appendage closure devices for prevention of stroke in patients with atrial fibrillation are considered investigational. Notice given 4/26/11. Effective date 8/2/11.
Tumor Markers	Policy separated out to Serum Biomarker Human Epididymis Protein 4 (HE4) and Urinary Tumor Markers for Bladder Cancer. Archive.
