Breath Testing for Heart Transplant Rejection Detection New policy issued. Breath testing for heart transplant rejection detection is considered investigational. Notification given 7/7/2005. Policy effective 9/15/2005.
Refractive Surgery Added verbiage indicating that the implantation of intrastromal corneal ring segments is considered investigational for keratoconus to the following sections: Policy, When not Covered and Policy Guidelines. Added new CPT Category III code 0099T (Implantation of Intrastromal Corneal Ring Segments) to Billing/Coding section. Code will be effective 7/1/05. Added key word and reference source. Notification given 7/7/05. Effective date 9/15/05.
Non-Invasive Measurements of Cardiac Hemodynamics in the Outpatient Setting Title changed from "Thoracic Electrical Bioimpedance" to " Non-Invasive Measurements of Cardiac Hemodynamics in the Outpatient Setting". Policy description, policy statement, when covered and when not covered sections expanded to include inert gas rebreathing technique. Policy guidelines revised to include rationale for noncoverage for thoracic electrical bioimpedance and inert gas rebreathing. MED1417, thoracic inert gas rebreathing, cardiac hemodynamic monitoring in the outpatient setting all added as key words. Notice given 7/07/2005. Policy effective date 9/15/ 2005.
