Title changed from "Thoracic Electrical Bioimpedance" to " Non-Invasive Measurements of Cardiac Hemodynamics in the Outpatient Setting". Policy description, policy statement, when covered and when not covered sections expanded to include inert gas rebreathing technique. Policy guidelines revised to include rationale for noncoverage for thoracic electrical bioimpedance and inert gas rebreathing.
