Under When Covered section - added the following statement to Guillain-Barré syndrome - "when presenting within 4 weeks of neuropathic symptoms if nonambulant and 2 weeks if ambulant". New HCPCS codes, Q9941, Q9942, Q9943, Q9944 added in Billing/Coding section of policy. Notification given 4/21/05. Effective date 7/7/2005.
