Blue Cross and Blue Shield of North Carolina accepts paper claims filed on the CMS-1500 version (08/05) claim form (for professional claims) or UB04 claim form (for institutional claims). These forms must contain all the required data elements, including (but not limited to): member identification number, NPI, date(s) of service and acomplete and accurate breakdown of services.
