Apply for a Provider Number
Providers numbers issued by BCBSNC are necessary to file claims for services provided to BCBSNC members. To receive payments made to the provider’s office, providers must have a provider number, must be credentialed and have a contract with BCBSNC. If a provider does not have a contract with BCBSNC, payments are made to the subscriber. Provider numbers can be obtained for individuals or for a group practice.
To apply for a provider number, please provide the following information:
Completed application for an individual or group practice
Download individual provider enrollment instructions
Download individual provider enrollment application
Download group practice provider enrollment instructions
Download group practice provider enrollment application
Completed Taxpayer Identification Number (W-9) form
Send completed application and Taxpayer Identification Number (W-9) form to the appropriate Network Management Field Office.
Please note: Completion of an application and receipt of a provider number does not authorize participation in BCBSNC networks. Providers must be credentialed and have an executed contract in order to be considered a participating provider. Please review our information on how to apply for credentialing.
