Notice to Participating North Carolina Medical Providers from Blue Cross and Blue Shield of North CarolinaBlue Cross and Blue Shield of North Carolina has attempted to notify by mail all participating medical providers that use their social security number as their federal tax identification number to inform them of a data breach involving personal information. The personal laptop computer of an employee of the national Blue Cross and Blue Shield Association in Chicago was stolen August 25, 2009. The laptop contained a file of the national provider data base containing providers' name, address, tax identification number and national provider identification number. Only the providers that use their social security number as their tax identification number are impacted. The Blue Cross and Blue Shield Association is offering one year of free credit monitoring services to providers impacted by this incident. Providers must enroll in this service by December 31, 2009.
