Help Close Cholesterol and Diabetes Care Gaps for Medicare Advantage Members

As 2013 draws to a close, Blue Cross and Blue Shield of North Carolina (BCBSNC) is making an effort to reach your Medicare Advantage patients with one or more care gaps. This outreach will focus on patients in the Cholesterol Management for Patients with Cardiovascular Conditions (CMC) population, as well as those in the Comprehensive Diabetes Care (CDC) population. 

Beginning this week, Carenet, a health care management services vendor, will begin making outbound phone calls on BCBSNC’s behalf to patients who had one or more of the following preventive care gaps during 2013:

  • LDL-C (CMC)    
  • LDL-C, HbA1C, and Nephropathy Screening (CDC)

A Carenet engagement specialist will educate these Medicare Advantage members about the benefits of scheduling their screenings. Once the member gives his or her consent, the Carenet engagement specialist will assist in scheduling an appointment on their behalf in an effort to close the care gap.
