BCBSNC to Adopt the New CMS-1500 Claim Form – Version 08/05

The National Uniform Claim Committee has released a revised version of the 1500 Health Insurance Claim Form (version 08/05) that accommodates the reporting of the national provider identifier (NPI). BCBSNC will adopt the use of this new claim form for professional billing purposes and will follow the implementation dates that the Centers for Medicare and Medicaid Services (CMS) has proposed for Medicare claims. The planned timeline is as follows:

• January 1, 2007: BCBSNC will accept the revised (08/05) 1500 claim form. (Note: Claims submitted on version 08/05 prior to this date will be rejected and returned.)
  
  
• January 1, 2007 – March 31, 2007: Providers can use either the current (12/90) version or the revised (08/05) version of the 1500 claim form:
  
  
  • Blue e direct data-entry claims will allow entry by either the new CMS-1500 format or the current format as of January 1, 2007.
    
    
    • April 2, 2007: The current version of the 1500 claim form is discontinued for paper and direct data-entry submission via Blue e; only the revised form is to be used. All rebilling of claims should use the revised form from this date forward, even though earlier submissions may have been on the 12/90 form.

BCBSNC is also planning to follow the guidelines set by the National Uniform Billing Committee (NUBC) for implementing the UB-04 claim form. More information on the UB-04 implementation will be release soon.
