New Information about State Members Enrolled in Blue Optionssm PPO

We would like to share with you more information about the new State PPO plan choices for State members enrolled in BCBSNC’s Blue Options PPO plan.

As you know, the State Health Plan offered their employees a choice of three Blue Options PPO products effective October 1, 2006. Under the banner name of NC SmartChoice, the three plans are NC SmartChoice Basic, NC SmartChoice, and NC SmartChoice Plus, and vary relative to the members' out-of-pocket costs under each plan.

To help you better understand how the new NC SmartChoice plan options are administered we have developed the following tools:

Contact Information and Summary of Benefits for State CMM, State PPO and Standard PPO Plans
Here you will find key contact information as well as a summary of some of the differences between the State Health Plan's current CMM product, the State Health Plan's new PPO product, and BCBSNC's standard PPO product. Please note the new PO box information for State PPO members.

State PPO / NC SmartChoice ID Card
Here you can view an example of a NC SmartChoice ID Card, which the new State Health Plan members will be presenting when requesting service. Please note that both the BCBSNC and State Health Plan logos are present along with the product name located under the Subscriber number.

State Health Plan EOP
Here you can view an example of a State Health Plan Explanation of Payment form, which will be utilized for NC SmartChoice processed claims.

Remember to check the State Health Plan member's ID card to verify in which plan the member is enrolled and don't forget to verify benefits and eligibility on Blue e sm.
