Patient-Centered Care Program
The Patient-Centered Care Program is available under your Service Benefit Plan coverage at no additional cost. The program provides you with one-on-one scheduled telephone appointments with trained pharmacists to help you better understand your drug therapy, reduce unwanted side effects, minimize serious drug-related problems and help you reach your drug therapy goals. The pharmacist will also answer any questions regarding your medications and related laboratory tests. Your pharmacist will send you a personal care plan that you can share with your doctor and other health care providers. Patient-Centered Care is dedicated to continuous care that helps you actively engage in managing your health.
