Comprehensive diabetes management

For people with diabetes, elevated blood sugars can lead to problems affecting the eyes, the kidneys and the nervous system. However, by learning to control blood sugars, these people can greatly reduce their risk of developing diabetes complications.

If you have diabetes, the Comprehensive Diabetes Management program can provide you with education, supplies and resources to help you manage your health, including:

Preventive reminders
We'll send you reminders for regular health screenings, including:

  • Annual dilated retinal exam
  • Annual Microalbumin urine test
  • Annual flu shot
  • Cholesterol test
  • Hemoglobin A1-C test

Educational materials
As a program participant, you'll receive a variety of educational materials designed to teach you more about living with diabetes, such as:

  • Information packets
    These packets include information on managing symptoms, diet and exercise, medications and testing. All materials should be used as supplements to the care you receive from your physician.

Blood Glucose Meter
It's important to test your blood sugar, but sometimes getting supplies can be inconvenient. With our Edgepark Surgical mail order program, you can get a blood glucose meter at no additional cost when you order a minimum supply of 150 test strips from Edgepark Surgical. Note: Your 20% coinsurance applies to the purchase of test strips. Edgepark will process all paperwork and obtain any necessary prescriptions or authorizations.

Support from a registered nurse
