Blue Medicare HMO and Blue Medicare PPO offer a variety of health and wellness resources for their members. There are no additional charges for these services.

• Health management programs
  

  Designed for members with specific chronic conditions, such as:

  • Congestive Heart Failure
  • Diabetes
  • Coronary Artery Disease
  • Chronic Obstructive Pulmonary Disease

  These programs feature an array of resources to help members manage their conditions. Each program includes educational materials, support from registered nurses, self-management supplies and more.

• Case management
  

  In certain health situations, members are eligible to work one-on-one with a case manager. A case manager can coordinate a member's care, help navigate the health system, monitor health care needs and more.

• Medication Therapy Management Program
  

  The Medication Therapy Management Program assists members who have been identified by the Plan as taking multiple medications for the treatment of certain medical conditions. The program helps members to understand better how to take their medicines safely and effectively. The Plan provides information to assist members to avoid potential medical complications related to their medicines and to help members take their medicines the way their doctor has prescribed.

• Telephone Learning Center
  

  Do you have health questions? Answers to your health questions are just a phone call away with the Telephone Learning CenterTM (TLC) Line. This toll-free health information line is available for members 24 hours a day, 7 days a week. You can choose to talk with a nurse or select from recorded information covering more than 1,600 health topics.

• Preventive care guidelines
  

  Review this list of guidelines to determine what kinds of immunizations, screenings and check-ups you should receive each year.

• Flu shot program
  

  Learn how to receive your free annual flu shot, a benefit of being a member.

Blue Medicare HMO and Blue Medicare PPO plans are offered by PARTNERS National Health Plans of North Carolina, Inc. (PARTNERS), a subsidiary of Blue Cross and Blue Shield of North Carolina (BCBSNC). PARTNERS is a Medicare Advantage organization with a Medicare contract to provide HMO and PPO plans. Plans are administered by BCBSNC. All qualified Medicare beneficiaries may apply. You must be entitled to Medicare Part A and enrolled in Medicare Part B and must reside in the CMS-approved service area. You must continue to pay your Medicare Part B premium, if not otherwise paid for under Medicaid or another third party.
